MISSION and OBJECTIVES
The mission /objective of Lake Country Area Agency on Aging Home Health is to provide quality, Medical Care which will promote independence and self esteem by assisting the elderly, ill and disabled in their homes. Thereby, establishing a continuum of long term care.
STEPS TO OBTAINING THIS OBJECTIVE:
Provide an alternative to hospitalization or other institutionalization.
Facilitate the recuperative or rehabilitative process for the patient.
Help prevent deterioration, recurrence, or complications of the disease
Promote the patient to an optimum level of health or well being than would otherwise be possible at the time.
Provide emotional support and medical care to the terminally ill patient and his/her family.
